INFORMED CONSENT
Please read the below information carefully
GENERAL PATIENT INFORMATION
Orthodontic treatment is a part of dental medicine concerning the proper development of the jaws, the alignment of the teeth in the dental arches and the removal and harmful habits such as mouth breathing, incorrect swallowing, tongue sucking, finger sucking.
This informed consent is designed to inform each Customer for the possible risks from the following products:
Impression Kit – trays and silicone material which you use to create the molds of your teeth and send them to us.
Aligner Kit - series of clear plastic, removable appliances that move your teeth in small increments. The product combines our doctor’s diagnosis and prescription with sophisticated computer graphics technology to develop a treatment plan which specifies the desired movements of your teeth during the course of your treatment. ALIGNERS or also known as transparent appliances are made of thermoplastic material by pre-taken imprint from your teeth. A series of individually crafted transparent, inconspicuous splints are used in the treatment with Aligners, which gradually move the teeth to achieve the desired result. The course of treatment involves taking impressions, crafting the aligners and wearing a series of individual splints tailored to the individual condition of the teeth at any given moment of treatment and may continue differently for different patients.
Before proceeding with transparent aligner splints, please be aware that your assistance and the care you will take before, during and after your treatment will be crucial. Treatment with transparent aligners requires patience, persistence and very good oral hygiene. Treatment with aligners can be approached due to both aesthetic and functional problems. Think carefully about your decision, we will be glad to help. Please be aware that aligners are not always the best solution for orthodontic tooth correction and are not suitable for patients with multiple distortions, bruises and serious orthodontic defects. Treatment with aligners is started after detailed orthodontic analysis using personal casts/3D virtual models, photographs, x-rays. Aligner treatment requires patient engagement, both during and after treatment.
Whitening Kit - peroxide or non-peroxide gel and LED- lightening mouthpiece aiming to deliver whiter and brighter teeth.
PROCEDURE:
Once you have made a decision to begin a treatment, you will have to take a print from your teeth that will allow the aligners to be individually made according to your specific needs and conditions.
Transparent aligners are made of suitable harmless material that does not affect your body. In order to get the maximum positive effect from the application of the aligners, you should strictly follow our orthodontists’ instructions; both for the placement and removal of the aligners and for the time you should wear them. It is strongly recommended that you remove the splints only when eating and brushing your teeth, with the maximum wearing time being determined by your doctor according to your treatment plan and you should comply with them.
The stages and number of transparent splints made depend on the individual patient and can be changed at the discretion of the orthodontist, and the time taken may also vary. That is why, if you have chosen aligner treatment, you should accept that the treatment takes time, which is different for different patients and can never be given a definite treatment deadline. The processes in the oral cavity of different patients evolve differently, which means that all the time frames you discuss with your doctor are not predictable.
REFUSAL OF TREATMENT:
The main risks of refusing the treatment are: staying in the state of your teeth or deepening and aggravating problems that already exist. You have to bear in mind that you can refuse to continue the already started treatment with aligners at almost every stage of it, but this will not lead to the desired result, even if the improvement is possible, it may be short term and in the long term deepen your existing problems.
RISKS
The BeeCorrect team will work to provide the best professional care to achieve the desired treatment results. As with all other orthodontic treatments, the Products in the Aligner, Impression and Whitening Kit may carry some of the potential risks described (not exhaustively) below:
Impressions
The most often risks associated with dental impressions are:
- Gagging is your body’s way of preventing choking. Outside of normal breathing and swallowing, when something touches the roof of your mouth, the back of your tongue, the area near your tonsils, or the back of your throat, a reflex contraction is triggered to prevent liquids and objects from entering your airway and throat.
- Accidental swallowing of putty material if it breaks off from the tray.
- Irritation and pain of soft tissue from the impression tray in higher pressure zones.
In rare cases you may show signs of allergic reactions. In such cases, please visit your GP.
It might seem simple to create a mold of your own teeth, but it is not as easy as it sounds. The accuracy of the initial impression or “molding” process is critical to success of the treatment as even minute inaccuracies are likely to result in poorly fitting aligners that will not work.
Aligners
- Wear the appliances for less then required number of hours per day, not using the product as directed by our orthodontists, can affect the ability to achieve the desired results;
- May be experienced teeth tenderness after switching to the next aligner in the series;
- Your gums, cheeks and lips may be scratched or irritated;
- There is a tendency for tooth movement similar to the initial deformation after treatment. Consistent wearing of retainers at the end of treatment should reduce this tendency;
- Periodontal disease, tooth decay, inflammation of the gums or permanent white spots (e.g. decalcification) may occur if patients consume foods or beverages containing sugar, do not brush and floss their teeth properly before wearing the BeeCorrect aligners.
- In rare cases especially if the patients have bruxism/ bruxomania, superficial wear of the aligner may occur where patients may be grinding their teeth or where the teeth may be rubbing and is generally no a problem as overall aligner integrity and strength remain intact.
- Teeth with short crowns, with atypical form, erupting, and/or missing teeth may affect aligner fitting and may affect the ability to achieve the desired results.
- Treatment of severe malocclusions, including skeletal jaw problems may require supplemental treatment in addition to aligner treatment.
- BeeCorrect aligners may affect your speech. It is possible to have lisp, which will disappear within 1 or 2 weeks.
- BeeCorrect aligners may cause a temporary increase in salivation or mouth dryness and certain medications can increase this effect;
- In order to create space needed for dental alignment teeth may require interproximal recontouring, depending on the shape;
- During the treatment you may have a temporary discomfort due to bite change;
- Supplemental orthodontic treatment, includes different appliances e.g. braces, auxiliary appliances/ dental devices (e.g. temporary anchorage devices, sectional fixed appliances), and/or restorative and surgical dental procedures may be needed for more complicated treatment plans. If oral surgery is required, risks associated with anesthesia and proper healing must be taken into account prior to treatment;
- Some teeth may be missing the gingival tissue below the interproximal contact once the teeth are aligned leading to the appearance of a “black triangle” space, because of inappropriate bone level between the roots
- BeeCorrect aligners cannot move dental implants
- General medical conditions and use of medications (e.g. estrogens, anti- epileptic drugs and so on) can affect orthodontic treatment;
- Current periodontal disease or/ and gum inflammation may be impaired or aggravated;
- There is a risk for useful life of previously traumatized or with large restorations teeth. In rare instances, the tooth may require additional dental treatment such as endodontic and/or additional restorative work and the tooth may be lost;
- Existing dental restorations (e.g. crowns) may become loose and require re-cementation or in some instances, replacement;
- The roots of the teeth may be shortened during orthodontic treatment;
- There is a higher possibility for breakage in patients with severe crowding and/or multiple missing teeth;
- The aligners are removable appliances and there is a risk of accidentally swallowing or aspiration;
- In rare cases, patients may show symptoms of TMJ (the lower jaw joint) disorder, causing joint pain, headaches or ear problems;
- Allergic reactions may occur;
- You may have a premature contact on the last teeth after the treatment if the aligners partially covered them. This will lead to supraeruption.
- In rare cases patients with a genetic disorder, may be triggered by mild stimuli including dental procedures.
Retainers
Wearing the retainers after the treatment may induce some of the upper mentioned reactions due to the similar qualities and composition of the material used for retainers and aligners production.
Whitening
The two side effects that occur most often with teeth whitening are:
temporary increase in tooth sensitivity
mild irritation of the soft tissues of the mouth, particularly the gums.
Tooth sensitivity often occurs during early stages of the bleaching treatment. Tissue irritation most commonly results from an ill-fitting mouthpiece tray rather than the tooth-bleaching agent. Both of these conditions usually are temporary and disappear within 1 to 3 days of stopping or completing treatment.
The degree of these side effects is directly related to the concentration of the peroxide bleach component, duration of the treatment, and the non-bleach composition of the product used.
There are additional risks that have been reported from in vitro studies which include tooth erosion, tooth surface roughening and softening, unacceptable color change of dental restorations, tooth mineral degradation, increased susceptibility to demineralization, and pulpal damage. The ultimate endpoint for tooth whitening is dependent upon the tooth itself, in some cases the final result is unsatisfactory.
Recognize that the risks and complications described are potential and none of them may occur.
The main risks of treatment with aligners are poor dental hygiene of the patient, impatience and lack of assistance from the patient, please consider when deciding on aligner treatment.
A successful treatment outcome cannot be guaranteed. After the final planned aligners have shipped, some cases may require refinement with additional aligners, traditional orthodontic techniques, and/or cosmetic procedures like crowns or veneers to achieve ideal results. There may be additional costs to you if you require such procedures. Always follow the directions for use (included with each Impression Kit and Aligner package) for best results.
With the present you are informed:
- that during and after your treatment, in order to maintain your condition, it is imperative that you follow all the Orthodontists’ recommendations and refrain from any action that may damage your teeth and health.
- that BeeCorrect may refuse to continue your treatment if we think that this is medically risky
- and understand the treatment considerations and risks presented in this form.
